I've been given this case to solve in an attempt to understand the topic of "patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with a diagnosis and treatment plan.

CHIEF COMPLAINTS 
A 50 years old female came to OPD with the chief complaints of fever(high grade),not associated with chills and rigors since 1 week. 
Admitted in hospital on 01/09/22.

HISTORY OF PRESENT ILLNESS 
She came to hospital with the complaints of fever, not associated with chills and rigors. 
Vomitings since yesterday,5-6 episodes.food particles as content.
Loose stools,3-4 episodes per day since 3 days.Dark colored stools are seen.no hematuria. 

HISTORY OF PAST ILLNESS 
Known case of hypertension since 1 year.

TREATMENT HISTORY 
Hypertension-on medication
No Diabetes,CAD, Asthma,Tuberculosis,hormones , chemo/Radiation ,transfusion , surgeries etc.

Occupation-Housewife
Appetite-normal 
Diet-mixed
Bowels-regular 
Micturition-normal 
No allergies 
No addictions 

FAMILY HISTORY 
No Diabetes , Hypertension , Heart disease , stroke,cancers,TB, Asthma etc.
